What we're looking for<\/h3>\n

  • Has at least two years’ experience in a care management role, ideally within a care home environment.<\/li>\n
  • Has experience of regulatory compliance and quality assurance.<\/li>\n
  • Possesses excellent knowledge of person-centred care planning and documentation.<\/li>\n
  • Has strong analytical skills with the ability to identify trends and implement improvements.<\/li>\n
  • Is highly organised with exceptional attention to detail.<\/li>\n
  • Has excellent communication and coaching skills.<\/li>\n
  • Is compassionate, proactive and committed to improving resident outcomes.<\/li>\n
  • Holds a full UK driving licence and is willing to travel across the South of England.<\/li>\n<\/ul>\n

        The role<\/h2>\n

        Reporting to the Regional Quality Manager, you’ll work closely with General Managers and Care Managers to ensure care plans are accurate, person-centred and fully compliant with regulatory requirements.<\/p>\n

        You’ll provide expert guidance, deliver training, conduct audits and support homes to continuously improve documentation and care planning standards, helping to ensure every resident receives the highest quality of care.<\/p>\n

        This is a hybrid role with regular travel across the South of England.<\/p>\n

        What you'll do<\/h3>\n
          \n
        • Support General Managers and Care Managers to ensure care plans are robust, personalised and regularly reviewed.<\/li>\n
        • Coordinate and oversee the electronic care planning process, ensuring records are accurate, compliant and up to date.<\/li>\n
        • Audit care plans and documentation, identifying trends and opportunities for improvement.<\/li>\n
        • Provide guidance to care teams on effective record keeping and appropriate care planning terminology.<\/li>\n
        • Produce quality and compliance reports for senior leadership, highlighting risks and recommending improvements.<\/li>\n
        • Support homes where non-compliance or quality concerns have been identified, implementing action plans to drive improvement.<\/li>\n
        • Deliver bespoke care planning and Person Centred Software (PCS) training to new and existing team members.<\/li>\n
        • Coach and support new Care Managers in their responsibilities for care planning and PCS governance.<\/li>\n
        • Promote best practice and ensure care planning aligns with regulatory requirements and company standards.<\/li>\n<\/ul>\n
